September Transfers

These were the cases transferred in September

1. Pancytopenia (later diagnosed acute leukaemia)

2. K/c AIHA for packed cell transfusion

3. TRO TB Meningitis: (later diagnosed viral meningitis with latent PTB)

4. Acute pancreatitis

From 7 cases down to 4… hopefully October have less than that.

Call upgrade

(Boredom alert! Merely a diary for It has been 4 months of working in this old environment but in a new post. Although still under observation, alhamdulillah, I am given the trust to fully function. It wasn’t very much different with registrarship in KL except that i get more phone calls, but would be able to sleep comfortably at home after midnight (so far…) 😊.

In January, I wasnt put in the roster for two reasons. Firstly, for me to “adapt” after 4.5 years of working elsewhere and secondly, the roster was already out when i registered 😁. And the following months I was given 3 weekdays and one weekend call. Heaven! Previously it was 6-8 calls per month for the first 3.5 years of training and 5 calls during registrarship plus lots of weekend team rounds. Now, most of my weekends are free except during oncall day 😍, which is like one weekend each month.

In general to date, i would receive 4 to 8 phone calls from the peripheries the whole day. Work will commence 8-5 as usual and towards the end of the day, i’ll take pass overs from the two intensives, which sometimes finish at 6pm. After dinner and rest for 2-3 hours at home and after Isya, ill head back to the hospital for night rounds in all 5 wards and peripheries till midnight… Thats a routine.

The first one was a bad introductory call whereby i received 11 referrals and finished night rounds at 2.30 am 😱. It was probably the worst night cause subsequently, it was much better.

Deaths so far, three, on average one each month will fall under my name. Sad… One sudden death of aspiration involving a 2 year old, another involving an infant with fulminant infection by metapneumovirus and the recent one was an extreme premature baby with pulmonary hemorrhage. I pray to Allah that all my calls are smoothsailing from here onwards.

Few more months to go before i leave for a smaller hospital in the hope of creating a huge impact on a smaller community. InsyaAllah.

I took up the chance to visit my Sri Aman abode by volunteering for a supervisory visit there and Betong.

It was an excellent feeling to come back and I was warmly greeted by familiar faces. Two main questions that were asked: “why lost so much weight!?” And “are you married!?”

Hillarious!

After seeing 72 out of 111 outpatient and 6 inpatients, we resumed our day with an hour of CME about a transfer checklist. I went home later to see a superficial defect in my master bedroom probably due to aircond leak😭. I ended the day with dinner with Kak Pija and family.

The next day was the trip to Betong. All 42 cases came and we ended at 1pm. After lunch, prayers, inpatient visit and CME, I headed home to Kuching which took 3.5 hours drive.

I felt more useful and appreciated in district setting. I guess id be happier in August 😊

Mandarin Class

Yesterday was very hectic. It was my first day at work, and first day of Mandarin class. I didn’t expect this new work life would be so hectic but with a very industrious consultant, things can turn from relaxed to frenzy! πŸ˜–

I finished work at 6.45pm only to hurry home, eat, shower, and rush to my 7.30 pm class. The class was fun! It was an introduction to new acquaintances; 9 students, some timid, but most are lively. We were introduced to vowels and tones on the first day.

Looking forward for next week’s class! πŸ‘πŸ˜Š

The Judgement Day

Onlookers, I was being unfair before for saying that the government seem to have a strange satisfaction of sending people away from their home. After the pep talk on 09122014 at Institut Pengurusan Kesihatan Bangsar, the new specialists were convinced that although the ministry try their best to accomodate everyone’s needs, their priority is the need of the people in general. They will post us, where our service is most needed.

IMG_6517.PNG That went on from 2.30 -4.30pm. And after a light tea break, its the Judgement Day for the pediatric department. Our big boss is a very considerate person. He gave the option of volunteering in critical areas, and for this period, one spot needs to be filled for Langkawi and 2 places for Kuala Terengganu. Langkawi was filled first, but there were no volunteers for terengganu despite big boss went round table asking each candidate. Everyone seem to have their destination of choice for placement. It took quite a while for him to settle it, and in the end he had to pick names to go. He chose, 2 of 3 male candidates who had never served outside of Klang Valley before. The ones who had spent years in Sabah, Sarawak, Kelantan or Terengganu will definitely get their place of choice, which i think is fair given such circumstances. But still, I pity the two victims. It was just unfortunate for them that there were no candidates from terengganu out of the 29 πŸ˜“. I was given Kuching/Sri Aman. Kuching for 6 months and Sri Aman thenafter. I was glad, because it was between that or Terengganu, Kuching/Limbang or Kuching/Bintulu or Kuching/Labuan! Sri Aman was the district i served for 3.5 years as MO before… So I shouldnt have any problem adapting. 😊. (And, there’d be less people to invite for my wedding… Whenever that is πŸ˜…)

Metamorphosis (part 2)

One caterpillar job done. Planned to complete another today. I managed to start my day early to catch the Head of Department to sign my project book, handed two copies for the department, one copy for my supervisor, rushed to Putrajaya and handed another copy for claim purposes.

I had lunch at BP near my home, prepared a passport sized picture to hand in on December 9th and went home to do my laundry. And after maghrib, i went to Meeples Damansara to join a game night with friends. We had an excellent introduction to Ticket to Ride, 7 Wonders and Citadel.
